Admissions to courses such as MBBS, BDS, B.Pharmacy, B.Sc (Nursing), B.P.T & B.O.T, B.A.S.L.P., B.Sc. (Radiology and Imaging Technology), B.Sc. (Radio Therapy Technology), Post Basic B.Sc. (Nursing), B. Pharm Lateral Entry, Diploma in Nursing, Diploma in Pharmacy, and other paramedical certificate/diploma programs are facilitated based on the marks obtained by eligible candidates in the prescribed science subjects at the qualifying examination, standardized to a base of 200 marks. The counselling process is meticulously structured to ensure transparency and meritocracy. For instance, in the Tamil Nadu NEET UG 2024 admissions, the DME conducted counselling for 85% of seats under the state quota and 100% of seats in private colleges. Eligible candidates were required to participate in the counselling sessions, where seat allotments were made based on their NEET scores and preferences. The process included multiple rounds, such as the initial counselling rounds and a mop-up round to fill any remaining vacancies.

 Candidates had to register online, fill and lock their choices, and upon seat allotment, report to the designated colleges within the stipulated time to confirm their admissions. The DME also ensures that the counselling schedules, seat matrices, and other pertinent information are made available to candidates in a timely manner through official channels, thereby facilitating a smooth admission process. Beyond undergraduate admissions, the DME is also responsible for the selection of candidates for postgraduate courses in medical sciences. This is accomplished through a Selection Committee appointed by the government, operating under the chairmanship of the Director of Medical Education.

The DME's comprehensive approach ensures that the selection process for postgraduate courses is conducted with the same level of integrity and efficiency as undergraduate admissions. In terms of infrastructure, the DME administers 24 government medical colleges and one government dental college in the state. Additionally, 50 hospitals and 13 dispensaries fall under its purview, highlighting the extensive network managed by the directorate.
